The “5-Whys process” is a common method to identify causes of the incident so that you can then take the necessary actions to prevent the incident from recurring. But what do you do if this technique only presents further symptoms rather than the real root causes?
This guide explores the benefits and limitations of the 5-Whys process and presents a useful method for further problem analysis. The method, which we have labeled as our PAR (preparation, analysis, resolution) process, is made up of 3 steps and presents a logical framework for getting to the bottom of any incident or accident in your workplace.
